This Privacy Policy explains what personal information we collect through this website, how we use and share it, and the choices and rights you have. It applies to visitors in the United States and to visitors from the European Union, the United Kingdom and other countries where our website can be accessed.

2. What we collect and why
2.1 Start Evaluation form
The “Start Evaluation” form asks for your name, phone number and email address, and questions about your tinnitus: the type of sound, how long you have had it, your level of discomfort, whether you have a diagnosed hearing loss, whether you use hearing aids, and your willingness to follow a 12-month program. You may also upload an audiogram.
This is health information. We only process it with your explicit consent, which you give by checking the acceptance box before submitting the form. We use it to assess whether the Levo System may be appropriate for you and to contact you with that answer. We do not use it for advertising and we do not sell it.
Levo Medical is a medical device manufacturer. Information you submit through this website is not protected health information under HIPAA, because it is not part of a treatment or insurance relationship. We still treat it as confidential health information under this policy.
2.2 Contact form
If you write to us through the contact form, we collect your name, phone number, email address, reason for inquiry and message, so that we can reply.
2.3 Email updates
If you subscribe to Resources updates, we store your email address and, if you provide it, your name. We send a confirmation email first (double opt-in) and only start writing to you after you confirm. You can unsubscribe at any time using the link in every email or by writing to us.
2.4 Browsing data
When you visit the website, our technical providers log data such as your IP address, device and browser type, pages visited, approximate country, and date and time. We use this data to run and secure the website and to understand how it is used so we can improve it. See section 5 on cookies.
3. How we share information
We do not sell personal information and we do not share it with third parties for their own marketing. We share it only with:
- Clinics and hearing professionals in the Levo Clinical Network, when needed to arrange your evaluation or treatment, and only with your agreement.
- Service providers that process data on our behalf and under our instructions: web hosting (WP Engine), content delivery and security (Cloudflare), email (Microsoft 365), email updates (MailPoet), customer relationship tools (HubSpot) and website analytics (Google Analytics and Microsoft Clarity).
- Authorities, when required by law, regulation or court order, including the U.S. Food and Drug Administration where applicable.
Microsoft Clarity session recordings mask all text you type; we cannot see what you enter in forms through that tool.
4. How long we keep information
- Evaluation and contact information: for as long as we are in contact with you and up to 24 months after the last contact, unless you ask us to delete it earlier or a legal obligation requires us to keep it longer.
- Email updates: until you unsubscribe.
- Browsing data: for the lifetime of each cookie, described in section 5.
5. Cookies and analytics
This website uses first-party and third-party cookies:
- Necessary: Cloudflare (security and bot detection) and the website’s own technical cookies. The site does not work properly without them.
- Analytics: Google Analytics 4, with IP addresses anonymized, and Microsoft Clarity, which records how the site is used (clicks, scrolling and masked session recordings).
- Marketing tools: HubSpot, which may set cookies to recognize returning visitors.
You can block or delete cookies in your browser settings. The website will still work, although some features may be affected. We honor the Global Privacy Control signal where required by law.
6. Your rights and choices
You can ask us to access, correct or delete your personal information, and you can withdraw your consent at any time without affecting the lawfulness of processing before withdrawal. Email info@levomedical.com and we will respond within 30 days.
California residents. Under the CCPA/CPRA you have the right to know what personal information we collect, use and share, to request its deletion or correction, and not to be discriminated against for exercising these rights. We do not sell or share personal information for cross-context behavioral advertising. The categories we collect are: identifiers (name, email, phone), health information you choose to submit, and internet activity (browsing data).
Visitors from the EU, the UK and other countries with data protection laws. Our legal bases are your consent (health information and email updates), our legitimate interest in running and securing the website (browsing data), and the performance of your request (contact and evaluation replies). You also have the rights to restrict or object to processing and to data portability, and you can lodge a complaint with your local data protection authority. Your information is processed in the United States; we rely on providers that offer appropriate safeguards, such as the EU-U.S. Data Privacy Framework or standard contractual clauses.
7. Children
This website and its forms are intended for people aged 18 and over. If a minor needs an evaluation, a parent or legal guardian must complete the form.
8. Security
We use technical and organizational measures to protect your information, including encrypted connections (HTTPS), access limited to the people who handle inquiries, and providers with recognized security certifications. No system is completely secure; if an incident affects your information, we will notify you as required by law.
9. Changes to this policy
We may update this policy when the website, the tools we use or the law change. The current version is always available on this page, with the date of the last update at the top.
